Make sure to hit the SUBMIT button at the bottom when you are finished. If your form was successfully submitted, a green box with a confirmation message will pop up on the screen and you WILL receive an email with all your responses. If you don’t get a confirmation pop up, please scroll through the completed form to make sure there are no error messages preventing it from sending.
